Your voice matters! The School Survey is a powerful way for you to share your thoughts about our school and directly influence its direction. Your feedback is crucial—it's used to assess our school's performance in the School Quality Snapshot and guides decisions made by our School Leadership Team.
3 Easy Ways to Complete the Survey:

  • Hard Copy: Fill out and mail the survey sent home in a green envelope a few weeks ago.

  • Online: Visit the School Survey site and complete the survey. (Please have your child’s OSIS number ready. If you need it, reach out to Tanica.). In the code field, in lowercase f followed by your child’s OSIS number (ie f123456789)

  • Online via NYCSA: Log in to your  NYCSA  account and click "Take the 2024 School Survey now!" at the top of the page, then scroll down to "Take Survey."

Important: If you have more than one child enrolled, please fill out a survey for EACH child.
