How to Fill Out Forms on Your Computer

  1. Download the form and save it to your computer.

  2. For the best results, open the form using Adobe Reader. This software is available for free download at: get.adobe.com/reader.

  3. Fill out, sign, and save the form. Tip: Rename the file to include the student's first & last name.

  4. Email the form as an attachment to your school: welcome@ps154.org
